What should I bring to my initial appointment?
You will need your physician referral, insurance card, photo ID, workers compensation claim number and case manager if it is work related, automobile claim number and adjuster if related to an automobile accident.
What Insurances do we accept?
We are contracted with most insurance providers in our area. We can verify your Insurance coverage when you call to schedule your appointment. If your insurance requires a copay per visit, copays will be required to be paid at the beginning of each visit. We accept cash, credit cards, and checks.
What do I wear for my appointment?
We recommend wearing clothes that are loose fitting and that you can exercise in as well as sneakers for in the gym.
How do I make an appointment?

How early should I arrive for my first appointment?
We recommend our patients arrive 15 minutes prior to their scheduled appointment to complete new patient registration.
